Hospice Care Basics
Our hospice team will communicate with the patient’s physician to discuss medical history, current physical symptoms, preferences for end-of-life care and life expectancy. Many components of our hospice care plan will quickly fall into place.
- Medications will be delivered to the home.
- The chaplain and social worker will arrive to provide emotional, psychosocial and spiritual assessments.
- The team will coordinate and schedule regular visits. Some will visit daily, weekly, when requested or as needed.
- You will receive information about how to manage symptoms and how to contact Elite Hospice if you have a question, if you need to see a member of the team, if you require acute care or if there's a crisis.
